Pre-Site Visit

Purpose

A pre-site visit before the site-visit allows health departments to correct any omissions early.

STEPS Site Visit Chair Only Site Visitors, including Chair PHAB Health Department

Assigns domains and "write" permissions to Site Visit Team Members.

NOTE: There can be one author only per domain.

     
2   Review Application [Application] and health department web site. Leave notes if needed.

Complete Pre-Site Visit Review [Doc. Submission/Measures]: Accept, Accept with Questions, or Reopen.

NOTE: Comments are required for Accept with Questions and Reopen.

The last Site Visit Team Member to complete his/her assigned domain will see a button [Home] to remind the Site Visit Team Chair to Review the measures.

NOTE: Home is always where the action is.

Select Accepted by Chair [Doc. Submission/Measures] from the drop-down menu on each measures page.

NOTE: The Site Visit Team Chair has an additional show/hide section, Site Visit Chair Review, on the measures pages. Open this section to access the drop-down.

Submit to PHAB.      
6     Review comments and questions and open measures where the site-visit team asked for additional documentation.

If additional documentation has been requested or questions asked by the site visit team, the health department receives a notification and has 30 days to respond.

      When additional requested documentation has been added, the health department director resubmits.
