Score measures and enter/edit comments in assigned domains under “Build Site Visit Report” section for each measure [Doc. Submission>Measures].

OPTIONAL: Request additional information [Doc. Submission>Measures].

NOTE: Steps 2-4 can recur several times.

Reopen measures requiring additional documentation.

If additional documentation has been requested and added to e-PHAB, the health department director resubmits.

Check off Ready for SV Chair Review for all assigned measures [Home].
 

NOTE: Steps 5-8 occur within two weeks of the site visit.

Review all comments entered by Site Visit Team Members, enter/edit comments, and finalize score for each measure [Doc. Submission>Measures].

NOTE: This step and the Build the Site Visit Report section create the Site Visit Report.

 

   
7 Check off each measure as Accepted by SV Chair [Doc. Submission>Measures].

Submit to PHAB [Home].

Edit Site Visit Report and send to health department director.

 

10

   

 

Review for factual errors and submit corrections to PHAB (within 30 days) (report is locked – only comment boxes are open).

11

   

Edit SV Report (Final Consensus box) based on health-department feedback, and forward report to the Accreditation Committee.
